Le corps de Girardia tigrina mesure entre 2 et 10 cm de long. Il est aplati dorso-ventralement et présente une forme ovoïde. Sa surface est lisse et recouverte de cellules ciliées.
Les motifs tigrés sur son corps sont caractéristiques. Sa couleur peut varier du brun clair au jaune, avec la présence de bandes sombres. Les yeux, situés sur une tête triangulaire, sont noirs et entourés d'une zone blanche.
